Incidents and hazards can be reported 24 hours a day, seven days a week by completing TSBC'S web form or by calling 1 866 566 7233 (select menu option #1 for our incident line). For immediate emergency assistance, call 911. Technical Safety BC protects the identity of persons who report or may be involved with incidents or hazards.

IUEC SAFETY STAND DOWN 2021

As vaccines are becoming more and more available and we navigate through the ever changing Corona Virus world, we must continue to re-enforce Safety on the job. While all of us are thinking about the Safety and Health of our families and friends, it is important to remind our Brothers and Sisters to focus on their Safety and the Safety of those working around them on the job. In order to again recognize Workers Memorial Day and The National Day Of Mourning on April 28th, the IUEC has prepared a Toll Free call-in number in order to hear a Safety Message from General President Christensen as well as links to a Life After Loss video and a video with Safety messages from several leaders of our industry including the IUEC, company presidents, employer safety directors and employer organizations supporting the Safety Stand Down. The call in number and links to videos will become active on April 28th.

KLEIN RECALL
NCVT-1 NONCONTACT VOLTAGE TESTER. If the button remains depressed during the power on or power off cycle, the tip of the tool remains illuminated in green, indicating the tool is "ready to detect voltage," when it is not.

2021-06-07 INCIDENT - CLOSE CALL - RIGGING FAILURE
While hoisting an elevator platform, the Hoist became dislodged from its supporting cable sling.

2021-06-07 INCIDENT - INJURY - STRUCK BY
While replacing a governor rope, a knife used to cut tape from the governor rope fell striking an apprentice in the back

While preparing to replace a synch rope on a hole-less hydraulic elevator, the elevator fell striking an apprentice.

A service mechanic responding to a report of an elevator making a strange noise, found a broken dead end hitch plate.

2021-03-18 INCIDENT - CLOSE CALL - STRUCK BY
Ice fell in hoistway approximately 15 floors after a mechanic and apprentice had stepped off the false car.

Upon testing a sky climber/false car and placing tension on the primary and secondary cables, one of the beam clamps (anchor points) detached from the overhead support beam (anchorage) and fell to the working platform below.

Linked is an is an IUEC Safety Alert regarding Uncontrolled Operation. The Close Call report is in regards to an Otis Elevonic 401 with an Otis 139 HT gearless machine roped in a 2:1 configuration. This should not be considered specific to only this equipment. 

Hazard: Uncontrolled movement of traction elevator

 

Notify supervisor when changing generator for prior elevator history and always use JHA checklist and write a detailed JHA.

CM 6 Ton Tornado Hoist with Latchlok Assembly, Stop Use and Inspection
Stop using the 6 Ton Tornado immediately & tag it out of service until the hoist can be visually inspected to ensure the retaining pin is installed. Failure to do so could cause personal injury or property damage.

While hoisting an elevator armature, the 4 wheel trolley being used came off the hoist beam.
